Choose this option if you're using regular paper (You can use white printer paper or plain sticker sheets)

*Avery template (Please specify the template number of the sheets you will use in the "note to seller" box at checkout or via etsy conversations)
Choose this option if you're using Avery sheets.

*Other
If you need a different size, please choose this option and specify the size you nee in the "note to seller" box at checkout or via etsy conversations

Text you'd like to include, template number or size (only if you're using Avery sheets or need an specific size)
